to help to improve the quality of life for our service users through certainty of support, quality of service and enabling them to remain independent in their home by providing additional care support where needed. Our goal is to deliver services that focus on the
mitigation of hospital admittances, behaviour change and self-care in order to maximise the ability of service users to live independently and safely, in their own home and within the community. We are committed to providing services that are entirely safe, effective, caring, reassuringly responsive and very well led.
